Job Description & Requirements - Specialty: Home Health - Discipline: RN - Start Date: 07/14/2025 - Duration: 14 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Travel ID: 54630389 Shift: Day shift - 8:00am - 4:30pm, Monday - Friday w/rotating weekends Description: RN Case Manager Home Health Full Time, Days Locations: Claremont, NH area White River Junction, VT area Visiting Nurse and Hospice for VT & NH covers more than 70 towns delivering superior nursing, rehabilitation, hospice, and personal care services with proven effectiveness, integrity, and compassion. RN Case Managers at the VNH screen, evaluate, re-evaluate, and treat patients as well as coordinate and communicate treatment activities with our interdisciplinary team members and the patient. The RN Case Manager: Develops a care plan in collaboration with the patient's physician, the patient, and their family, or the primary caregivers upon admission. Periodically reviews the patient care plan and recertifies the patient care plan as appropriate. Coordinate the patient's care plan through interdisciplinary communication and case conferences, as appropriate. Advise other disciplines of discharges Minimum Qualifications: Must be a licensed Registered Nurse in Vermont and New Hampshire or hold a compact license. Graduate of an accredited school of nursing.
