Job Description & RequirementsRN Case ManagerStartDate: 6/9/2025 Pay Rate: $2000.00 - $2700.00Position Summary - LcswPOSITION DUTIES - Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care PlansMinimum Required Qualifications -

• MA RN license
• BLS
• 2 years of case management or utilization management experience and 3 years of medical/surgical nursing care experience.
• Basic computer and typing skills.
• Effective verbal communication skills and problem solving and conflict resolution skills.
• Demonstrated ability to organize and work independently.PREFERRED QUALIFICATIONS - Bachelor's DegreeLENGTH OF ASSIGNMENT - 13 weeksSHIFT / HOURS PER WEEK - Usually M-F 8-4:30.
