Position: Nurse Navigator

Department: Belkin Breast Health

Schedule: 40 Hour Days

Position Summary

Serves as a liaison to the Breast Health Program and Belkin Breast Health Center among patients, families, referring and consulting providers, and affiliated departments to expedite patient access to appropriate, timely breast health services. The Nurse Navigator plays a critical role in the development and maintenance of a state-of-the-art breast center serving the patients and providers from internal and affiliated practices of Boston Medical Center. Coordinates all education that is needed in order to reach a decision about trial enrollment for eligible participants. Provides documentation in the electronic medical record, and is knowledgeable about cancer care.

Assists in triaging incoming Breast referrals:

* works with New Patient Coordinator to triage referrals
* reviews medical records
* Works with New Patient coordinator and staff to obtain all clinical, imaging, and pathology reports; films and pathology slides as necessary to ensure smooth evaluation at the time of appointment.
* Facilitates the flow of information between patient, clinician, and other services related to breast health care
* Collaborates with Lay Patient Navigator to ensure patients are tracked and followed in a timely fashion.
* Works under the supervision of the operations manager to ensure the patient and family satisfaction goals are met.
* Establishes and maintains excellent collaborative relationships with referring and consulting providers and staff; affiliated departments (medicine, surgery, radiology, pathology, medical and radiation oncology, patient financial services, etc.).
* Assesses the patient and family in a systemic and ongoing manner for both overt and covert signs and symptoms in accordance with the standards of clinical nursing practice.
* Collects pertinent data using appropriate assessment techniques.
* Involves the patient, family/significant others and appropriate healthcare providers in the collection of data when appropriate.
* Consistently uses clear, concise and effective written/oral communication and complies with departmental policies when sharing/documenting relevant patient care data.
* Demonstrates use of critical thinking skills, derives and documents appropriate diagnoses from patient and family assessment data.
* Effectively delegates appropriate patient care activities to co-workers with appropriate knowledge and skill level and ensures that co-workers have completed the delegated tasks in a timely manner.
* Provides for a safe, clean unit environment through use of appropriate risk management practices, e.g. labeling an removing broken/expired equipment/materials for service.
* Educates patient and family regarding current illness, procedures, and treatments, plan of care as well as health maintenance, promotion, and illness/injury prevention.
* In emergency/crisis situations, demonstrates effective decision making in accordance with standards of clinical nursing practice, re-prioritizes patient care needs, initiates appropriate response actions, and directs other team members to ensure a complete and coordinated response.
* Documents interventions according to departmental policy.
* Conducts systematic and ongoing patient reassessments and evaluations.
* Provides follow-up care in person and via telephone when needed and when appropriate.
* Functions without supervision in a manner appropriate for level of experience and education.
* Responsible for seeing things through to completion and for informing appropriate staff of outcomes.
* Performs post-surgical teaching and wound care
* Demonstrates cultural competency with respect to the patient population served.
* Maintains accurate annual professional development record.
* Demonstrates flexibility and ability to positively adapt to changing staffing/clinical/patient census needs including but not limited to offering assistance to others, adjusting patient assignments, floating to other areas/units, etc.
* Maintains a presentable appearance in accordance with dress code requirements including wearing a visible identification badge that identifies one's self as a registered nurse.
* Completes the following mandatory requirements: annual BLS certification, institutional mandatory education, e.g., safety regulations, infection control standards, etc., and departmental/unit specific competencies.
* Acts as a patient advocate in order to facilitate meeting identified patient needs.
* Delivers care in a non-judgmental and nondiscriminatory manner that is sensitive to and demonstrates respect for patient diversity.
* Seeks available resources to help formulate ethical decisions.
* Communicates and consults with client, significant others and members of the health care team regarding required patient care.
* Works cooperatively and constructively with all departments and health care team members to establish an environment that supports the process of collaboration.
* Develops and fosters relationships with clinical staff to become a resource for patients.
* Completes annual mandatory safety training and PPD testing.
* Seeks guidance when necessary.
* Performs other duties as assigned or as necessary.

EDUCATION:

Graduate of an accredited nursing program, BSN strongly preferred.

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:

Licensed to practice as an RN in the Commonwealth of MA-required. Requires current basic life support (BL) certification. Additional certifications may be required.

KNOWLEDGE AND SKILLS:

Clinical expertise and ability to work independently to make appropriate clinical decisions within RN scope of practice. Experience in clinical supervision of non-licensed support staff. Ability to apply critical thinking, clinical and administrative problem-solving skills to solve problems as they arise regarding referrals and other departmental issues. High-level computer skills required for use of EPIC. Clinical oncology experience is essential and experience working with cancer clinical trials as well as an under served population is preferred. Excellent communication and customer service skills, ability to coordinate multiple detailed tasks in a professional manner.
